Application Form 報名表格
Please fill in the information clearly in BLOCK LETTERS. All the certificates will be printed according to the English name written in the application form. Please check the information clearly.
請以正楷清楚填寫資料，所有參與證書及奬狀將根據報名時所填寫之英文姓名印製，請清楚核對資料。
Online application is for Individual participants only. Participants must complete the artwork label and stick it on the back of the artwork, then send it with the application form and entry fee (cheque / bank-in slip / copy of FPS) to Room 1001, Chinachem Johnston Plaza, 178-186 Johnston Road, Wancha, Hong Kong before the submission deadline. Please indicate “Tomorrow Art Pioneer 2020 International Creative Competition for Children” on the envelope.
網上報名適用於個人參賽者。參賽者可於網上報名填寫表格及上載參賽費用(支票/入數紙/轉數快副本)。遞交網上報名表後，繪畫比賽之個人參賽者需下載「參賽者作品標籤」， 列印並貼於作品背面， 於截止報名日期前將作品郵寄至本會: 郵寄至香港灣仔莊士敦道178-186號華懋莊士敦廣場1001室， 信封面註明「明日小小藝術家國際兒童創意比賽收件組」。
Individual participants of Clay and Mixed-media categories, please submit the online application form and upload 5 photos of the artwork altogether. The actual artworks is not required at this stage.
參加黏土或混合媒體比賽的參賽者只需填寫此網上報名表及上載作品相片5張，暫時不用遞交作品。
Application Deadline 截止報名日期：31/1/2021
*If applicant choose to pay by cheque, please choose "Cheque", and send the cheque together with the artwork to us by mail.
* 如參賽者選擇以劃線支票方式繳費，參賽者需選擇『劃線支票』，並於郵寄作品時一併連同支票遞交。
